Public Health Referrals in cc Link (Outpatient)
Click “Meds & Orders”
Type “Public” in New Order search-the 8 Public Health Referrals appear
Other Public Health-only use if the client does not fit in any other referral type PH Adult Referral-use for homeless adults with chronic medical issues PH Child Health Referral-use for Pediatric clients for assistance with referral for developmental/specialty follow-up PH Communicable Disease-use for all types of communicable disease (TB, HIV, and reportable diseases (i.e. shigella, brucellosis, etc.) PH Lactation Referral-client’s with lactation issues that need Lactation Consultant and/or PHN HV. PH Perinatal Referral-use for pregnant women, Postpartum, Perinatal Depression PH WIC-Nutrition Referral-(not for lactation) for general WIC referrals, or special formula orders.
